February 13, 2008	Wednesday	**Learning Plan and Time Sheets due to Faculty Field Instructor

*Students are to be in placement for 15 hours weekly over 16 weeks for a required total of 240 practicum hours.  Mondays, Wednesdays, and Fridays are the days designated for field practicum.  The student abides by the holiday schedule of the practicum agency!

**When completing the Agency-Based Mid-Semester Progress Report and Agency-Based Final Evaluations, the student and field instructor should prepare their own independent assessments and share with each other their written perceptions before completing the final report to send to the faculty field instructor.  Both the student and the field instructor must sign the forms before sending to the faculty field liaison.  DO NOT fax the forms to the school.
